Please read the following agreement carefully. By signing below, you acknowledge that you understand and accept the terms of participation at Iron Forge Athletics.

Assumption of Risk, Release & Waiver

INFORMED CONSENT AND ASSUMPTION OF RISK

I, the undersigned participant, hereby acknowledge that participation in CrossFit, functional training, strength and conditioning programs, HYROX training, personal coaching, open gym sessions, and any other activities offered by Iron Forge Athletics (the "Activities") involves inherent risks, including but not limited to: muscle strains, sprains, fractures, cardiovascular complications, overexertion, dehydration, and in rare cases, catastrophic injury or death.

I understand that these risks cannot be completely eliminated regardless of the care taken by coaches and staff. I voluntarily assume full responsibility for any and all risks of injury, illness, or property damage associated with my participation, whether foreseen or unforeseen.

MEDICAL CLEARANCE

I represent that I am in good health and have no medical condition, injury, or physical limitation that would prevent my safe participation in the Activities. I have consulted with a physician prior to beginning any exercise program at Iron Forge Athletics, or I have knowingly chosen to participate without such consultation. I agree to inform Iron Forge Athletics staff immediately of any changes to my health status.

RELEASE AND WAIVER OF LIABILITY

In consideration of being permitted to use the Iron Forge Athletics facility, equipment, and services, I agree to release, waive, discharge, and hold harmless Iron Forge Athletics, its owners, officers, employees, coaches, contractors, and affiliates from any and all claims, demands, liabilities, or causes of action arising out of my participation in the Activities, including claims based on negligence, except where prohibited by law.

INDEMNIFICATION

I agree to indemnify and defend Iron Forge Athletics against any and all claims, suits, or actions of any kind brought by or on behalf of any person arising from my conduct or participation, and to reimburse Iron Forge Athletics for any damages, costs, or attorney fees incurred as a result.

PHOTO AND VIDEO RELEASE

I grant Iron Forge Athletics the irrevocable right to capture, use, and publish photographs, video recordings, and audio recordings taken during my participation for promotional, marketing, and social media purposes, without compensation or prior approval, unless I provide written notice of objection.

GOVERNING LAW

This agreement shall be governed by and construed in accordance with the laws of the State of Texas. Any legal action arising out of or relating to this waiver shall be brought exclusively in the courts of Travis County, Texas.
